Broadly speaking, derived by various commentators from the Torah, not always quite identically. They are all valid today, but not all have an application today, mostly because the temple is not still standing.

They are all in the Torah. See Rambam, Ramban, and others. If you get a (five-volume?) copy of Sefer haHinnuch, ascribed to R. Aharon haLevi of Barcelona, you will see them listed in the order in which they appear in the Torah. That book contains explanations of each mitzvah, and applicability. Much shorter compilations are available, usually as Sefer Mitzvot, or Taryag (Hebrew gematria for 613) Mitzvot.
