I think (therefore I might be) that they put them outside so they could read the meter, but I don't know if that's necessary anymore.

Mine is outside. I had to replace it in 2009 and I was told that was code.

Our house is new construction (2015) and has both an exterior panel, and an interior sub panel. I suspect the code requires an external main shutoff, probably to protect first responders and the electrical company workers.
