On the back of the form, it says "two (2) of the following forms of id must be shown: State Issued Driver's License or id, us passport, military id, dd214, naturalization certificate, birth or baptismal certificate, social security card, or ins authorization to work." Photocopies not accepted..
So is this to say that a social security card and a OHIO drivers license will be accepted? The birth certificates are in Ohio and won't be here for a week and we would like to get this out of the way asap.

If I have had a gaming card before and it was suspended due to an incarceration about 10-12 yrs. ago, why is it that the Sheriffs Department will not allow me to acquire another until I go before the Gaming Commission Board? Then they wonder why las vegas has many returned inmates!! They make it purely impossible for a person like myself to get their lives on that right and normal path. What can I do? Who can I talk to, e-mail, or even drive and be personal, face to face, to help me?

not much....there are too many people out there for the Casinos to want to gamble on a formerly encarcerated person....[casinos are NOT in the business of gambling]

do what they ask and go before the Gaming board.....or not....your choice
