I went to Riverhead for an appointment today and definitely would consider Riverhead a rural area, far removed from the hustle and bustle I’m familiar with (I live in Western Suffolk and have spent lots of time in Nassau).

At what point do the suburbs/influence of NYC/feasibility of commuting to NYC end and “Out East” begins?

My cut off is Sills road/ 101. After that you still have suburban areas but they tend to be further spaced apart or more run down.

Hmmm... I would say basically William Floyd Parkway as the dividing line on both NOrth and South shores..

Which is not to say that I'd personally ever consider commuting to NYC from anywhere east of Babylon or Smithtown, LOL

Probably the eastern side of Town of Brookhaven. Starts to transition around Manorville-ish.
