Dishonorable Discharges are handed out at court martial for serious crimes like murder, rape, desertion, etc. They can not be given for discharges for medical reasons, performance reasons, or other things that might lead to a discharge.
If you actually have a dishonorable discharge, depending on your state, you may not be able own firearms, vote, etc. It is similar to a felony. You don't get a DD for nose bleeds.
