The one down the block is Russ and Daughters Appetizers which has been there for over 100 years I believe. They're so famous and historic that they're opening a sort of museum/restaurant on Orchard Street soon.
I was reading about a little place on Jerome Ave nr Montefiore that just opened that serves corned beef and pastrami sandwiches in the style of those delis. I remember a kosher style deli restaurant on Jerome years ago
Katz is not cheap and overrun by tourists, but it does hold a sweet spot in my heart and I've always enjoyed the food there.
Ben's Best on Queens Blvd in Rego Park also has really good food. The last time I was there (2 months ago I think) I was surprised by the prices. I felt like they were charging Katz prices for a Queens experience.... Food was good though.

That was also named Katz's and it was the first place I had a hot dog with potato salad. Great place as I remember from my childhood. Norwood Boy must remember it as well. It was near Jade Garden.
when I go to Montefiore Moses I am annoyed at the lack of American restaurants in the area. I like Spanish food and other ethnic food but these days its not good for me because its too acidic. The Greek owned diner on Jerome and GunHill closed down (great food), the Jewish places are gone, it turns out that the hospital cafeteria is just about the only place to get American food around there...
I did forget to mention the Court Delicatessen in 161 and Walton. ordered a brisket on rye piled high for lunch while on jury service. I think it was only $6 and change. high quality too

Probably a lot less crowded than Katz in Manhattan.
Ben's on Queens Blvd. is the only one left in the area. So I guess they feel that they can charge the higher prices. In general kosher restaurants charge more as they have to pay the rabbis for their kosher certification besides their regular expenses.
I would avoid Ben's Best in Rego Park at all costs. Not only because the food is subpar and nowhere near the quality it should be for the prices they charge, but because a family member of mine had a horrific experience there years ago. He almost died because of the neglect of food safety there, and wasn't even offered an apology afterwards.
thats scary. im always worried about that too. i just walked away from a latino place on White Plains Rd that only had a "B" rating into Subway which had an "A"
