I moved to another state last year for a job which required me to get a new driver license from that state. When I got the new license my NC one was hole punched and they let me keep it. Recently I moved back to NC with same address I had before and driving records show my NC license is still active. Apparently they do not communicate that I got a new license and to cancel the old one. I rather use the NC one since my permanent address is in this state. Would there be any problem if I just stop using my old out of state license and re-order a duplicate NC one to use?

If it allows you to order a new one (with the proper address), there is no problem. You should cancel your one in the other state, though.

This kind of thing is how people end up registered to vote in more than one state, too--there is little if any communication between states even when someone is honest about being registered/licensed in an old one.
