We live in DFW metro in Texas now but getting tired of sky high property taxes and the weather is too warm for my taste. Ive explored TN on the ground and extensive research as well as Greenville area in SC.
Key needs / wants beside the items mentioned are equivalent medical access to what we have now. Wife has RA and had Lupus so we run into lots of unusual medical issues and need access to great RA Dr and Concierge Internal Medicine Dr. Being without 30 to 45 miles of Medical needs is close enough.
We prefer suburbs to the city but not small town fans unless they are suburbs of a large metro. We are not fans of being near water so higher elevations where its cooler appeal to us more.
We grew up in small town Oklahoma but over the past 50 years have lived in suburbs of Houston, Pittsburgh, NYC, and now DFW, Tx. We like having access to good restaurants and a variety of grocery needs as we have now.
Ashville is way too small (metro size matters to us more than city size) Greenville was borderline and only Nashville in TN was large enough to peak out interest. I know Duke has a top national reputation for RA patients so being with in a coupe of hours would be ideal. So sounds to me like west of Charlotte might work if there are some great suburbs that fit my destruction.
Any guidance on where I should investigate further in NC?

Duke is in Durham so about 2 1/2 hours to an area west of Charlotte. You might want to consider the Winston-Salem area. It would put you an hour closer to Duke and they have a good hospital system there, as well (Wake Forest Baptist). I think Clemmons and Kernsersville are popular suburbs. But those more familiar with the area may be able to give you better suggestions.
I did that last move but families move and age too. My home has property taxes if $15k per year. If I didn't have have a senior exemption it would be $22k vs no more than $10k where I'm considering. I've family in TN and NC as well as NM, NY and Oklahoma. Also in France but not a consideration for us

I pay zero income tax so the amount is different in each State that has income tax. I have calculated my income tax, property tax and any other meaningful tax rates and know NC is high cost in taxes to me than the other locations I am considering. I also calculate the fed tax impact so look at the cost of SALT after fed income taxes on my situation. All locations I am considering are over the $10k cap on deductibility on federal income taxes.
My plan is to buy a home of comparable value and know my income for the foreseeable future and car strategy too so estimating income after federal and state income taxes has been done. I am not looking at any cities that have their own income taxes as none of those locations are of interest to me. We lived back east in Pittsburgh and NJ during my working career so have some experience with high tax regimes/states.
