I am driving my son to a medical apt. We have to be there at 10a.m. From what I can tell there are 2 options:

1. 66 to 495 to 270
2. From Arlington through Bethesda (Rockville Pike) to 270.

How long will this trip take, roughly? Is one route preferable over the other?

66 to 495 to 270 is the far better route. However, why not take the GW Parkway to 495. That would be even better in the morning than either of the 2 routes, if you're coming from certain parts of Arlington.

Definitely take 495 to 270. You do not want to be on Rockville Pike - it is all stop and go traffic and lots of lights. I commuted up to 270 not far from Shady Grove for many years. Once you get onto 270, it is smooth sailing in the morning. Everyone is going the other way (south on 270). Even on 495, it's not usually too awful - slow sometimes, yes, but not too bad.

ETA: From Tysons Corner near where I live to Shady Grove Hospital it should be about 45 minutes.

As dkf747 mentioned, take the GW Parkway west to 495 and then 270 There's less traffic going west on the GW Parkway than going into the city. No stoplights and great scenery!
