Manoa Falls. Easy hike. But be prepared to freeze your butt off when you enter the water. Most visitors, haoles especially, think the water in those pools are as warm as the ocean waters.
Wrong!

La'ie Falls, Maunawili Falls, Manana Pool. Kawainui above Hale'iwa is outstanding, but access is very limited and nearly impossible to obtain for anyone outside of an established hiking organization.

A good place to start would be the State Dept. of Land and Natural Resources trail info site:
Na Ala Hele - Hawaii Trail System: Oahu Trails

Hiking enthusiast Dayle Turner has a site that list trails by geographical area: Oahu Hike Tales (by Area)

The HI Trail and Mountain Club and the Sierra Club, HI Chapter are also good sources of info on hiking in HI:
The Hawaiian Trail and Mountain Club [HTMC]
Sierra Club Hawai'i Chapter, Oahu Group

Default Pools

I would advise not swimming in any of the pools on Oahu because of leptospirosis. There are many hikes which go back to waterfalls and pools. It is best in the winter. Many of the pools can dry up in the summer.

While the number of cases nationwide of leptospirosis is highest in Hawaii, also note that we are one of only a few states that reports identified infections.

Leptospirosis Causes, Diagnosis, Symptoms, Treatment and Prevention on MedicineNet.com

While not swimming in freshwater pools is the only sure way not to contract leptospirosis, in reality that is rarely going to happen. Swimming clothed, not drinking the water and minimizing contact with your mucous membranes, avoiding post-flooding waters or stagnant pools where concentrations of rodent and mongoose urine are likely to be higher are reasonable preventative measures.

I have been hiking and swimming in freshwater pools all over Hawaii for over 3 decades in the company of many people from local hiking and environmental groups, and have yet to see a confirmed case of lepto amongst them. Leptospirosis is out there, but in comparison to say, driving while talking on a cell phone or not taking your prescribed high blood pressure meds, the level of risk is fairly low.

Just discovered Manana Trail and it is AMAZING. I've looked at the hills and wondered if I'd ever get a chance to be on top of one. It's 4 miles in, though, and I wasn't totally prepared for that so I didn't go back to far. I'll be returning with my game face on. <-- that ain't it.

Dragon's Nostrils, side of Makapu'u. Tide pools at the bottom are very nice. It's not the lush tropical rainforest type of hike, more of a desert hillside type. But the pools and the blowholes are pretty awesome. If you go midweek when school is in, odds are good of being the only one's there.
