I know this is a long shot, but while going over my expenses and receipts I came across one that I can't remember where it is from. Any idea how to track it?
Adams forms are "stock" forms sold at office, resturant, hospitality, and just about every supply house in the US and abroad, so they are not identifiable just by the numbers. You will need to rack your brain for where you were that you got a receipt and which places are you missing receipts for. That SA540a is one of the most common selling tens of millions each year.

Is there a date on the receipt? I would think it is for a restaurant. Did you pay cash?
Is it possible you've already been reimbursed for this expense and just the bottom got torn off, which you've now come across? I'd check old paid records to see if the receipt number matches to a top missing a bottom. But only if this is critical in some way. It might not actually be worth the effort and best forgotten.
You were served by someone with the initials M.B.
The number is just unique to the piece of paper like a check number. It doesn't mean anything to you except when you pick up your order.
Sorry, figured out it was from the snack bar at the Drive-In last weekend.
