I am trying to determine the stock values of Chemical Bank stock which was purchased through an employee stock purchase plan from 1994 through 1996 when the bank merged with Chase Manhattan Bank and later JP Morgan Trust to determine the cost basis. This this stock was sold in 2017 and I want to file an amended tax return based on the cost basis but am unable to determine the actual purchase price. I need the historical stock symbol so I can look up the historical value at the time of purchase.

Get a NYT or WSJ during that period, should be one on microfilm or hard copy at your library. Stock price will be listed in business pages.

Check the SEC for annual filings?
Email your current broker and ask
Email the stockholders dept at the current bank and ask.
