Pretty clear. He is changing the meaning of "we" between the first and second clauses of the statement.

Payouts to malpractice suits are less than 1% of your healthcare dollar.
But the insurance to cover the doctors is HUGE. And gets passed down.

49% of the populace pay nothing to support the federal government now, why would we believe that is going to change. Do we really think the government will force poor people to pay? MassHealth requires families make more than 150% of federal poverty guidelines before they have to pay monthly premiums and that's after deductions.

well said. The GOP talking points (remember Bush...."doctors cant spread their love ...with women patients")...

"tort reform" is a shills talking point they throw at the masses, to pretend it is an issue they even care to address. They don't want the system changed for a reason.

average americans end up filing bankruptcy when they get any long-term illness, while members of Congress get premium care at the Mayo Clinic, all on the taxpayer's dime.

I assume this is the quote?

"It is amazing that people who think we cannot afford to pay for doctors, hospitals and medication somehow think that we can afford to pay for doctors, hospitals, medication and a government bureaucracy to administer it."

Rebuttal: We're managing to pay for all that now. There is no free healthcare and we have plenty of bureaucrats. What makes anyone believe we can't do it in the future?

Meaning, Sowell's sarcasm is accurate?

That's a "rebuttal"?
