Honestly, I think there are far many more mishaps than we're being told. It wouldn't shock me if they occurred relatively frequently, too, or that they always have. But that's a part of being human. Thus, while security policies can always be reviewed and possibly updated/changed, by and large the service doesn't need to be changed in my view, at least not fundamentally, though some policies can be tweaked (you know, things like locking the White House door, etc.).
The Secret Service used to be the one federal LE agency with a pretty good reputation. I agree with Tex, you have to be able to fire people. Fire one, and the rest will be scared straight. Also it sounds like the supervisor who covered for the drunk agents should be gone.
Also forget about political correctness when it comes to this job. In the last case where the mentally ill guy got inside the wh, they had a lady agent guarding the door, and the man overpowered her.

They should shoot him or release the dogs.
I'd disband the Secret Service altogether and turn over the protection of the President to the Marines.
The SS don't have a clue what discipline and service and honor are all about. The Marines will do the job they're given.
