I believe one of factors is illegal Immigration. Many former middle class American males had jobs doing construction and those jobs were paying decent wages. With the influx of low wage workers who are willing to work for wages which do not allow one to survive in the US, it's simple supply and demand and American workers are paying the price.
Add the fact that housing costs have gone up but not wages and we have a death spiral. The choice is to be homeless or dead. The illegal workforce live in flop houses which allow twenty to thirty men living together. Americans are not welcome to live with them and IMO are not welcome on job sites either.
Being disabled I tried to have a friend and his wife live with us and it took less than a month for my neighbors to report me to the town yet other houses owned by slumlords only a few blocks away have so many men living together but the town does nothing.
One more thing, if American labor does not speak Spanish and the other workers do not speak English then tell me how can they work together? For example my brother in law worked with his dad until he had a stroke and could not work anymore. He tried to work in a local factory but only lasted one week due to the Spanish speaking workforce. He was the odd man out in his own country.
I believe this has a lot to do with the rise of drug and alcohol abuse and suicides. Of course it is just one piece of the puzzle many other factors are involved.
I think you have a point, but that's only a small piece of the puzzle. You're ignoring the mass exodus of manufacturing jobs from the US which have left former middle-class towns as shells of their former selves, where dollar stores, Wal-Mart & Taco Bell are often the only means for gainful employment. I live in one of those towns, and the heroin epidemic, not to mention other health problems like hypertension, diabetes and obesity, runs rampant.
Have middle-aged working class whtes become the most marginalized group in America? No affirmative action for them, no politicians promising special programs, no calls to "do something for my people". But there is heroin...
One other factor has not been looked at: Geography.
What part of the USA is the death rate the highest? I notice that in the South, the death rate among White and Blacks is higher than than elsewhere in the USA.
I subscribe to the believe that you die when it is your time. Obviously, these people, for the most part, abused their bodies. It has nothing to do with class or education. Most of these studies don't amount to much and the government usually funds them. Most of them down a lot of prescription drugs: More Americans Than Ever Use Prescription Drugs - NBC News and others eat themselves into ill health.
Though this thought won't be popular, when religion played a big part in the lives of people, you didn't see as many druggies and alcoholics. I find my peace not through addictions but through my faith in God.

The elephant in the room; no pun intended is Obesity.
These people are literally eating themselves to death. Just look pictures of people 50 years back and now. Combine that with literall no physical activity, drugs and painkillers and this comes as no surprise.
Those complaining about immigration and loss of manufacturing jobs ain't seen nothing yet. Wait till robotics and artificial intelligence kicks into high gear in the next 10 years.
You won't have to worry about Jose to compete for jobs with; worry about R-800 and iRobot.
And T-1000s will be ready to quell any unrest.
What will really be needed to deal with the surplus population, and which smacks dab of "socialism", is massive wealth redistribution and guaranteed minimum income.
It's either that or the Terminator scenario.
