The cake would be the same service, but I'm not sure the procedures are the same for medically necessary and elective abortions.

They generally are. The uterus is surgically invaded and the fetus is removed.

Pork is legal, but restaurants can serve bacon and not be required to sell pork loin.
Popsicles are legal but stores can offer strawberry and not sell raspberry.
Alcohol is legal, but bars can offer beer and not sell whiskey or wine.

Wedding cakes are wedding cakes. There is no difference in the ingredients, baking times, or colored icing. If he did not offer two groom toppers, then he didn't offer groom toppers, but this case is not about the decoration of the cake or the topper. He is refusing to sell a cake to a gay couple that he would sell to a hetero couple.

Actually, there is a difference. This couple bought a gay pride wedding cake with rainbow layers, elsewhere.
