Which of these city duos, that sit on opposite ends of the state feel so different, that you would almost feel like you are in a different state?

As a Floridian and former Missourian who has been to all the states on your list, I have to go with Jacksonville and Miami as being the most different. The rest have some minor differences between the sister cities, but the difference between Miami and Jax is far more pronounced.

shouldnt Miami to Jacksonville actually be Key West to Jacksonville or Key West to Tallahassee?

Seattle to Spokane would be legit as states have actual size out west.

we could add something like Sitka AK and Barrow AK for giggles.

Jacksonville is a politically and culturally Southern city and Miami is the polar opposite.
A little nudge to the north and Jax would be in Georgia.
Nobody would notice the difference.

The title specifically states medium to large cities.

Miami to Jacksonville. No way in hell would I live within 200 miles of Miami but I would consider Jacksonville.
