Before I answer, I want to double check- you need skilled nursing versus assisted living? Assisted livings do a LOT of medical care, granted not as much as skilled nursing, but I've been surprised to see how much medical they do (there's only one I can think of that seems to shy away from a lot of medical care). I have way more assisted living suggestions, but if it's skilled nursing you need I can probably come up with a few. I frequent those facilities daily for my job.
I'd recommend Magnolia Glen in Raleigh. There are two separate buildings; one is an independent living with available assistance if needed while the other is assisted living where the resident essential has a live-in nurse.
Be aware that you do not get to make the choice on the facility. Suggest you do a bulk mailing of the FL-2 to every NH in the state, and then see which one(s) contact you if they can accept the patient for admission.

That is if your loved one has Medicaid, I believe. Otherwise, you should have pretty good choice as long as the place you are interested in has beds open. Actually, even with my folks with Medicaid, I still have a choice as long as more than one responds back to the FL2.
I regularly visit someone at Springmoor - it seems to have a fairly nice feel to it. They have independent and assisted living. Good luck - a difficult decision, no doubt!
I suggest that you visit nursing homes of interest to get a feel of what actually goes on inside. Visit several times including all shifts, weekends, and at meal times.
I did that when I was looking for a place for my mom.
I liked Sunnybrook and her stay there was satisfactory, not perfect, but good. I understand that they have made some changes within the last few years, so I don't know how the facility is run now.
Also, check the state site for NH and review any reports made. That was a big determining factor for me.
