Not always so.
You do not need to be drunk to wind up on the wrong side both on freeways and one way streets.
Still remember going to start electric service when I first moved here. After an hour in the office, I left the parking lot. No sign on the driveway indicated a one way street. I wound up going the wrong way.
Quite a few freeway entrances and divided roads are poorly marked.

One way streets are one thing. One way highway entrances? Those are a little different. I've seen a couple that are maybe sort of confusing but they are pretty well signed as far as I remember.

I've seen some on freeways too.
One was where Walnut St intersected with 440 in Cary. You could easily turn right thinking you were on the next street and wind up the wrong way on 440.
Happened once to a lady in a minivan. No freeway traffic made it easy to mistake the offramp for the next block.
This was fixed in a redesign, but there are more such bad intersections and not all are fixed.
How about this..........A sensor in the pavement that reacts to a vehicle going the wrong way and it activates a flashing red NO ENTRY< WRONG WAY sign ? That covers the argument about " I didn't know it was a one way ramp " .
Of course there are other more serious ways.......tire shedders/ spikes that rip the tires off the rims, but only if the vehicle is going the wrong way. If you are moving in the correct direction the spikes stay below the road surface. Used at most US border entry points. Follow the correct traffic direction...no problem.

my neighbor handles a large insurance company's auto claims.
according to her, there are three major reasons for wrong ways:
1. Impaired. intoxicants, age or disabilities (left glasses at home).
2. Immigrant. legal or illegal. first year driving in the States.
3. Inattention. looking at smartphone instead of the road.
