If he was working on your furnace without the flue pipe presently installed, then I can see where it could be easy to accidently install it that way. BUT, once he put the flue pipe up and noticed the furnace slot was in the wrong place, then he should have fixed it. The original mistake wasn't as bad as him having had to notice the problem before he left your house. That's the problem.
I'd leave the picture out of the review as it's meaningless to probably 90% of the population, instead explain what happened and that it was rectified once you pointed it out to him. For me, that would be enough not to use him. I don't want to have to correct contractors' mistakes. He did correct it without issue, sp maybe that's good enough for some people.
When I am reading reviews for anything I want just the facts, no emotion...let people make their own decisions.
He fixed it immediately. Move on and forget about it.
