Why in the world would you voluntarily try to have an office romance? They rarely end well for either one involved and can cause lots of unnecessary turmoil, drama and someone getting terminated.
IF you think the risk is worth it then ask the woman out however, I don't understand how you could possibly know that she was hurt that you did not return her smile after you stated you didn't notice she smiled at you when you were busy with someone else.
