Last time in a restaurant was 1970. I didn't even own a credit card and all transactions were cash.
I'm taking a friend to lunch. When ready to leave, I hand the credit card to the waiter. I want to have the tip included. Do I simply say, "Include a 20% tip."? Or is there a more appropriate protocol?

OK. You seem to be saying that I do state up front when I turn over the card, the percent I want to tip, and then the waiter brings it back for signature.

No, you never have to mention the tip to the waiter.

You write the amount of your tip (in dollars and cents, not in percentage), total it, and sign at the bottom. You leave the signed copy and the pen on the table, and take the copy with you for your records.

In Canada, every restaurant brings the little portable credit card machine to the table so that you can do it all yourself electronically. Is that really still not happening in the States?

In some places, yes, but generally no.
I'm also curious about how someone has not been to a restaurant in almost 50 years.
