Continental but their flight attendants are the absolute rudest bunch of people I have ever come across. Their planes are clean and fares are still kind of cheap.
You are so right. The flight attendants are the rudest ever, make you want to rethink if the fares are worth. I wouldn't mind paying a little more if I am going to be treated better.
Stateside, I tend to fly US Airways, since I fly to Charlotte a lot, and they're the vast majority of Charlotte's flights. AirTran has given me lower fares to Charlotte, but they've burned me too many times. I'm done with AirTran.
Im a devoted fan and member of Southwest, for the shorter flights, like maybe one state over or whatever. But for the longer flights, I like United, or Ted (same difference).
In economy/coach class:
Domestic: Frontier
International: Virgin Atlantic, Emirates
In business:
Domestic: none (I'd rather fly Frontier coach than business on UA or AA.
International: British Airways (granted, haven't flown business on Virgin though).
I am *shocked* that noone in this thread has mentioned JetBlue! Best airline ever! Only problem with JetBlue is they don't fly to ATL any more. Otherwise no airline comes close.

I tried looking up Jet BLue once but at that time, couldnt find anything for the location I was going, so havent really tried since.
My top tier would be various Asian airlines: Cathay Pacific, Singapore Airlines, and JAL. All give a level of service those of us in the US can only dream about when we fly domestically.
In the middle tier I would put British Airlines. A good level of service, but a notch below those above.
The bottom tier I would put various US airlines. At the top of that, I would put Southwest, JetBlue and the like. These are better than other US airlines, like American, Delta, and so on.
There are worse airlines out there, though. Some of the cut-rate European airlines are absolutely terrible. But I avoid those like the plague now.
In the U.S.: Jet Blue (they fly out of our local small airport here in Long Beach, CA), I also have had some really good experiences on Song Airlines which is Delta's budget airlines - like Jet Blue, they have television screens on the back of each seat and a menu of food from which to choose - Song Airlines actually had a few delicious gourmet type salads a nd sandwiches, cheese with fruit and crackers, etc....lots of leg room and good sized seats in both Jet Blue and Song.
Emirates!! Best service, best food, flight attendants are friendly and helpful no matter how demanding customers are.
