They did a re run of this earlier show. She has not seen him in prison, but he has written to her. Is a psychopathic killer capable of caring for a family (genuinely) while also being a sadistic killer?

Probably to some degree, but I doubt it's the type of love a normal person would feel for their family. Having a wife and kids helped him maintain a totally normal appearing façade, while he committed his crimes. He actually comforted his wife and daughter by telling them they had nothing to fear from BTK, and he was the only person on the planet who could truthfully make that statement.
If he truly & deeply cared for his family, he would have stopped killing and never been caught and no one but him would have ever known. No, I don't think he was capable of true love. It would have to have outweighed his sexual fantasies, and that didn't happen.

The family members suffer differently. The victims and their families bear the brunt of it though.
