I have never done one. I plan on giving myself all 4 out of 5. I will list examples why I think so. Am I rating myself too high or too low. 5 means I am the perfect employee. 4 is above average and 3 is average. My boss can agree or disagree when she signs off
These things are so stupid. The point of a review is to determine if the employer ie your boss is happy with your work not if you are happy with your work. That is like the doctor giving himself a customer satisfaction survey. Typical HR crap.
I just did mine yesterday. Blah blah blah. I hate them.
Also, I guarantee your manager doesn't like them any more than you do. There's always some idiot who rates himself a 5 when he is actually a 2 and the manager has to give him the bad news.
Lol, I love the comments above and am thankful a lot of other folks consider these as frivolous as I do. I'll bet I've said essentially the same thing every year for the past 20 years, just in a different way each time.

Read the guidelines, and describe your performance in those terms. For example, if 5 is the highest and the guidelines say something like "consistently exceeds goals and expectations" then list examples of when you did that.
Depends on the culture where you are, but you probably have to describe yourself in glowing terms about how you saved the word, cured cancer, and ran a marathon all in time to have dinner on the table. Evaluating employees is a manager's job and pushing it off on the employee is one thing I can't stand. I never did it to any of my staff.
I don't like doing the self assessment; I don't like having to do the reviews of my employees. That being said, I have asked a few employees to do the self assessment to see how they perceived their work (mainly because they had, in my view, a few "needs improvement" ratings. It gives me an opportunity for understanding. Even though they have ongoing feedback throughout the year, the annual reviews by me are required, and I feel I have to be honest on their reviews.
