Talk with your manager and ask him/her what needs to be done, to be promoted to a Sr. role.
Show him that your are interested in taking on more responsibility and are interested in a promotion.

In technical fields at an individual contributor level, I'd say at least five years - maybe ten in more conservative organization. If you have ten or more years of progressive experience and aren't there yet, you need to start looking.

Not really. The company could hire someone from outside.

Most of the times, it is about who you know, not what you know. It is important to have experiences and good technical skills; but if you don't know the right person to go to, don't have social skills, don't know how to talk, how to sell yourself, you won't get the promotion you want.
