We use foot warmers on the batteries. They have a sticky side unlike the handwarmers. We just stick a few of them on the battery packs.

All you need to do is use two or more Lithium-Ion batteries and rotate between an inner pocket (ie, warm) and the camera.

When batteries are cold they cannot deliver the charge, but they are not discharged and do not need a recharge. Just warming them up is all it takes, and your body heat is quite sufficient.

Another trick is to wear a coat that is large enough to put the camera inside when not using it. That keeps the whole camera warmer and means it will take longer for the batteries to get cold. Obviously with a small Point&Shoot camera that is much easier than it is with large DSLR cameras; but I manage to do it with a large Nikon DSLR even with a 70-200mm lens attached.
