Lisa,
One of the utmost of importance items on your list I would think would be the cardiology departments of the local hospitals, if they could meet your needs and a local cardiologist that could take you and your health plan. Perhaps you have already gotten a referral here? Medical care is a bit different and specialists are not in abundance. Some people have to fly into Honolulu to see a specialist.

Ahualoa is a beautiful area. One of my coworkers lives there and loves it, but she does face the "commute" back and forth to town each day. I know, I know...it's such a nice drive. But, when you are basically "on call" all the time as she is, it can become an issue. We've considered living there too, but probably not until the day comes when we can comfortably work from home most days (in other words, not any time in the near future)!
