Personally instead of any city, I associate the Grand Canyon the most with AZ. Otherwise Phoenix.

Kinda depends on the context. Arizona from a traveling perspective, Flagstaff or Kingman. Arizona as just a place someone I'm meeting is from, or in a political/cultural context, Phoenix

I feel like this question should be reserved for states with more than just one or two notable cities... basically, Texas, Cali, Florida and maybe Ohio... that's about it.
