They would be a hero when they're saving your life. Also some doctors don't get paid or cut their pay in a disaster. Finally, firefighters and cops can be heroes, and they're "getting paid".

Hero is for something that they do beyond their call and risk themselves to save others.

But not all people being paid to do things are equal. A firefighter/cop might go above and beyond what is normally done to help and this could be considered heroic.

Even soldiers who put their life at risk are not considered hero otherwise we won't have very few receiving medals.
Yes, their job is heroic due to nature but just because of that doesn't make each a hero.
You have to do above and beyond what you signed up for to be considered something more up to being a hero.
"Oh my god, you are a hero" definitely comes out if a doctor fixes a broken leg but that is not a hero.
