That's leaving out a few black market type drugs likely account for some of those deaths, but I get your point. I have a tendency to blame prescribing doctors as much as I do the drug companies, maybe more so.

From different documentaries I've watched, the common denominator is that at some point, the user is prescribed pain killers due to some injury or whatever. They then become addicted to the prescribed pills and after the prescription runs out, they end up looking for other means to get that same high.

Is there no personal responsibility to discuss with their physician that there is a problem?

I take you have never talked with anyone who is addicted. Denial is one of the first things they come up with. You ask, they will say that they don't have a problem and that they can stop whenever they want.

The vast majority of people for whom opioids are prescribed never become addicted.

South America and Mexico are the major source for heroin in the US and a large amount of opioids come through Mexico so street drugs probably play a big part in these deaths.

From my understanding it is pain pills that cause heroin addiction and people are ODing all the time.
