It sounds like liver issues to me and more specifically a fatty liver which does indeed cause fatigue and does come with high triglycerides. One indicator of a fatty liver, if it's advanced, is elevated enzyme levels.

Many years ago I had a test that showed I have a fatty liver. I was embarrassed because at that time, fatty liver meant alcoholic.

I wasn't and am not an alcoholic. So was relieved to see other things cause it.

I'll have to research that. Thanks!!

Fatty liver is not uncommon in people who gain weight after middle-aged hormonal changes. You may have noticed how some men and women gain weight around the middle. Some men get kind of a barrel shape. A lot of this is due to visceral fat they gain, including in the liver. People who haven't had a drink in their life can get fatty liver. IMO giving people the hormonal support they need as they go through mid-life changes (if HRT isn't contraindicated for them), is key in preventing this weight gain and visceral fat build-up, which makes people vulnerable to a number of other diseases, including diabetes.
