I would have also found it weird that people would assume everyone around the world spoke English.

However, my personal experience is that some Americans have not been overly exposed to travel outside of the United States and have never given it much thought.

A quick illustration. One of my best friends and I were in Cologne, Germany. I have traveled a lot, while this was his first visit to a non-English speaking country. We had stopped in a busy coffee shop for a snack and were trying to work through the crowd at the counter to get to the exit. He got a irratated when after saying "excuse me", people did not react and let him through. It just did not occur to him that they did not understand what he was saying.
