NOT counting fancy places where you (or somebody) paid an arm and a leg. Just regular affordable places. (Its not my intent to limit this to North America, but it might be most helpful if it lists places we can all get to)

2. The Livestock auction barn, Erickson Nebraska
3. Strouds (North, the original)), Kansas City MO
4. South City Limits, Traverse City MI
5. Brush Country Steakhouse, Pearsall TX
1 - Pappa's - Houston, Tx--Wonderful fish dishes, just great fresh seafood all around
2 - Landry's - Kemah, Tx--All of their seafood is great and great ambiance with the boats going by
3 - Ralph & Kacoo's - Shreveport/Bossier, La
4 - Pappadeaux's - Kemah, Texas
5 - Szechuan - Nacogdoches, Texas
1 - Quivey's Grove, Verona, Wisconsin -- Good, solid Midwestern food with a uniquely Wisconsin flair, in an old farmhouse. Creative seasonings and flavor combinations, unparalleled presentation, seasonal changes in the menu, attentive and friendly waitstaff, and best of all, thoroughly unpretentious.
2 - LuLu's Restaurant, Madison, Wisconsin -- Simply superb Middle-Eastern food in a deceptively plain setting. Chicken shawarma to die for, heavenly lamb stew, wonderful kefta, fabulous hummus bel-lammeh, and I think my daughter could eat her weight in their yalenjee.
3 - The Old Feedmill, Mazomanie, Wisconsin -- Another place that does basic Midwestern fare extremely well and with great creativity. Their grilled walleye pike is unbelievable.
4 - Smoky's Steakhouse, Madison, Wisconsin -- The archtypical Wisconsin supper-club atmosphere, which means completely unpretentious and down-to-earth, generous servings, waitstaff that have worked there for decades, and really, really, really good steaks.
5 - Rossario's Restaurant, Monona, Wisconsin -- Another one that has the classic Wisconsin supper-club atmosphere, but with amazingly good Italian food instead of steaks. The basic menu by itself is exceptionally good, and they always have a couple of specials that are outstanding. The most modestly priced of all my selections, too; a meal and a glass of decent wine for two people is usually around $30 or so.

Another I would have added to the list (if it was still open) is Regents Row, which was in Queens, NY for many years. They had the best American bar food.

That stuff can not be called "Chili". Its beef with gravy over noodles.
