Some examples in Boston:

Good: Faneuil Hall Marketplace, The Shops at Pruedntial Center
Bad: Copley Plaza Mall, Cambridgeside Galleria

All four have rail accessibility, but only Faneuil Hall integrates directly and well with the surrounding streetscape. Both Copley and the Galleria are so inwardly oriented that there is no relationship to the city at all. The Prudential escapes this problem by providing useful pedestrian connections between a few different neighborhoods in what are long, glass covered promenades, which wind there way around buildings and large stores, filled by small stores and carts as you move from building to building. It is much more the feeling of an outdoor market, and wherever you enter or exit, you are right in the midst of pretty good destinations.

My favorite is the Montreal Underground

Sure none of them have good street presence, but still pretty cool
