Housing affordability will be the toughest nut to crack, as nice places to live tend to attract more people. Boise is a good choice.
Smaller would be Walla Walla, WA with it's growing wine scene (~100 wineries), three small universities, and being on the boarder with Oregon... the opportunity to shop sales tax free. There is pretty good medical options, and very good if you have VA access. The mountains in Oregon are close to the south, so snowy mountain views are possible, and if you want a day in the snow it's close by. The town gets a few small snowstorms each winter. Also the Snake and Columbia Rivers are nearby, plus the snowy mountains means water is pretty secure. Most of the racial diversity will be white and Hispanic, but there are some from other races. There is a bus system (Valley Transit), but I'd guess it's not super extensive, but likely somewhat functional if you are going to/from major destinations (colleges, shopping district, hospitals, etc.). No major natural disasters likely in the area either. Bigger Tri-Cities WA is about an hour away for more extensive shopping and better air travel options. It's worth a few minutes of internet searching at least.
Just to add to this...OP, I noticed that you mentioned in another thread that you like snow. So, I'd say to look into areas near the Great Lakes in Upstate NY and NW PA(and in the Midwest), which get their share of snow and fit the vast majority of your criteria.
Buffalo comes to mind for its underrated art scene, affordability, multiple hospitals(inc. Roswell Park), walkability in multiple neighborhoods and select villages(Kenmore, East Aurora, Orchard Park, to a lesser degree Williamsville, etc.) and more.
Rochester's SE Quadrant has neighborhoods such as Park Avenue, South Wedge, Monroe Village, Upper Monroe and East Avenue, among others and adjacent Brighton also for close proximity and has walkability around Twelve Corners and along Monroe Avenue into the city.

Attempting to thread the needle, I would say the Minneapolis-St. Paul area. The taxes aren't low, but you do get something back for what you put in (unlike a lot of high tax places).

So, that would mean a place where they are not plentiful, with a low COL.

This does not answer my question. I did not ask which points on the list are valid, useful, practical, or anything else.
Just curious and if you don't mind me asking, but why are you looking to leave Maine?
I might be thinking of leaving Maine. Or I might not. If I do entertain the idea, I am curious about which places might meet as much of the items on my list as possible.
