um the healthcare issue is a real thing. if you have anything serious, you must be airlifted to oahu for decent treatment, and to the mainland for specialized treatment. the outer islands basically have clinics only, whose services are overtaxed. Slower response time in treatment sometimes means people die in transit.
living on the outer islands is probably the equivalent of living in a very rural area on the mainland where you'd be airlifted to the hospitals in more major cities.
