Thankfully the OP’s husband recognized the pill, but sometimes a pill can look different (due to different brands of the same generic) and people might not immediately know something is off if they get a different looking pill.

OP- glad your husband recognized the medication.

how do you see that working? in the states there is plenty of automation and mistakes still happen. mistakes will always happen. there's always going to be a human somewhere in the process.

Unfortunately,it happens.

I have been known to miss something on busy, distracting, days and dispensed the wrong med..Not often, thank God, but once is too often

Today's problem is that we have to stop and give a vaccine, or council a patient, or show someone where the Tampax is located, or talk to a doc that prescribed the wrong med, or , or , or

If I got the wrong meds I'd be pretty mad.

Humans make mistakes. Unfortunate it happened, but you caught it.

Which should be illegal, IMO.

Manufacturers can stamp their pills with their logo or whatever, but pills should have a standardized shape, color and size in the name of safety.

Back when my dad was alive he was given the wrong prescription and the pharmacy actually realized it and sent someone to his house to retrieve it. I don't remember now what it was but it was something that could have had very bad results if he had taken it.

Not only that but part of my job involves working with a lot of instrumentation and that instrumentation regularly breaks...
