Birth control dates back to something BC so if they knew when to use it they probably knew what caused it.
The earliest forms of birth control, as well as abortion, were found in Ancient Egypt and Mesopotamia as far back as 1850 BC. What is interesting, is this has been the basis for much of the discrimination against homosexual people. The idea, or belief, that sex and having children were always connected, and understood, which isn’t really the case. That sex is justified through reproduction, when in reality 99% of sex is for pleasure exclusively.
People have sex because of hormones, specifically testosterone, that ignite sexual attraction/lust, the need for close intimacy, and an outgrowth of love two people feel towards one another. The baby produced was just a byproduct of the sexual interaction that took place. Supposedly there are still tribes that exist, that still do not connect sex and babies being produced; the theory that only the “gods” bring babies, exclusively to women.

9 months after first sex act? Or maybe after 10th or so intercourse?
Actually, I suspect they watched other animals and figured it out. It takes us 9 months but other animals have shorter gestation periods -- rabbits about a month. It might have taken a while longer to figure out fertility cycles.
