It's 5% and 7%. The tile and the mastic both have asbestos, and one is 5% and the other 7%, although I can't remember which. It's chrysotile.

There is concrete under it. Aside from having some mastic stuck to it, it's in good shape. There's only one crack if I remember correctly.
As far as I can tell it's not letting moisture in. I will need to tape some saran wrap in various places on the exposed concrete to know for sure though.
