OK, I don't even believe that. We give out visitor visas like candy. There is something they don't like or some part of the process she has not completed to their satisfaction. Perhaps we had reached the numeric threshold for visa issuance for that particular country each time she's applied.

Green Card Lottery is one option. Student Visa is another option.
Have they visited US in the past?
Geniune Tourists have their own money and strong ties to their native country. They dont stay away for months and also dont require documentation that all their needs will be cared by someone else. Normally people with money, good job, strong ties are not denied Tourist visas but they should have it in their native country.

No caps exist, no quotas exist. People from that part of the world have a bad habit of immigrating themselves on a visitor visa. The rate of fraudulent applications is high. The consulate is under no obligation to issue non immigrant visas.

If she has a college degree, if you have a business, you invite her to your firm and give guarantees. She can get a Green Card and later try to get the test for citizenship.

If she doesn't have a college degree recognized by the US, she can still join your LLC but I think you should post an ad in the papers, by law, and accept a few candidates in your office, review their CVs, like 4 or 5 of them, and deny them, and deny them with a believable reason and explain to the immigration officers why you denied these local Americans and why the Ukrainian girl/man is better than them. What skill does this person possess that Americans lack! You better be sure they'll believe you or you can end up in a Federal prison for fraud.

Then you answer for her basically. She does something stupid, you burn too. And you pay the fees for all this too, yeah. $2-3K it was I reckon.

Then she would live in your state, your city, work in your company, for the span of 5 years, or more, until she gets a citizenship. Or she will have to work for you forever. Or go back.
You may want to do some research on employment based US visa.

Well, that was certainly an about face. What's it going to be, America is too dangerous or come anyway and take your chances getting maimed or killed in a police shooting?

No visas are simple to get coming into the US and the lottery is insane difficult; there is also a very definite cap depending on the country you are coming from for the lottery and it varies each year.

There really doesn't need to be a reason for denial but the thing that would stand out most to me would be the time visiting for just the first visit. Having assets and employment do show ties to the country but the question as to how someone could leave a job for that long and why could be a doubt to IO's. My husband was fortunate to be part of the visa waiver program but he still was pulled aside at poe each visit and asked to show all ties to his country, ownership and amount of time to be in the US. We were told specifically that the first few visits should be kept short. He didn't extend more then 10 days to 2 weeks each visit for the first years until the fifth visit when he stayed the summer. Each stamp that he got made it easier at poe thus assuming this is the sentence advising to travel more.

Advise to visit them in Ukraine first and leave this for awhile then try again but request a shorter time. You can also show photos from US visitors in their home and can have a letter written as well. While this still could be denied, if you get someone whom can maybe see the ties and the situation, it can be approved. Lastly, seek professional help in this. Get a consult for a local lawyer and show the denial letters.

Best of luck. We are dealing with trying to get a visa for a longer stay for my husband's mother here in the US as she is in her 80's and poor health but so far it has been a long process and we don't hold our breath on this. My husband and I have lived back here in the US for 5 years now, married almost 10 and together between the countries 12 years. She owns properties all over Italy but, with all this, we are still running into problems and my husband is resigning himself to hoping to hear of her funeral when she dies now and hoping to get to her before then as she's all alone there.
