Depending on where you live in MD, you can go over the border into PA near Gettysburg... they have a great Apple Harvest Festival in Biglersville,PA. It is usually the 1st 2 weekends in October.
The Catoctin Mountains usually have some type of fall festival as well....

Agreed - Garrett County is THE PLACE to be for autumn in MD.

Ditto the comment on Garrett County. Nearby WV is beautiful as well, especially south and west of the Oakland area where the elevation is still pretty high.
