Given that there appears to be a wide divide between what people desire their health insurance plans cover, I thought I would ask everyone what their ideal health insurance plan would be. If you could design your coverage, what would it cover?

I will try to be as impartial in the poll choices as possible, but if none of the choices fit your "ideal", please choose other and elaborate. Please, keep in mind that the more extensive the coverage, the higher the premium; the less extensive the coverage, the cheaper it is.

Also, please state what you would be willing to pay for said coverage.

Personally, I prefer lower monthly premiums and believe health insurance should be modeled after the car insurance industry. People are more informed health care consumers when they are part of the process. If your doctor orders a bunch of tests for you, you're more likely to question the necessity of said tests if you are responsible for the first care dollars spent.

I would choose a cafeteria type of plan where I can choose what types of coverage I don't have the need for. My second choice would be a major medical plan with a low monthly premium.
