Maybe. But Baby Kim could simply have high blood pressure and a weak heart. Even that could be a lie, and Kim put it out just so he could go quarantine in peace and quiet.
North Korea is a black hole where no light escapes from it. Espionage on the ground is next to impossible there, so no one knows what's going on with Kim. His own people don't know any more about it than the rest of us.
Kim has a big advantage in that. He was raised in the West, mostly in Switzerland, so he knows and understands how the west thinks. Kim Jon Un understands the powers of deception very well; he learned them from his father, his grandfather, and his uncles and aunts from the minute he was born.
Covid hit South Korea, so it hit North Korea too.
Kim has tried every trick to get his way he ever learned from his family except for one; none of them ever had the opportunity to play dead.
Kim and his little sister have always been very close, according to all the most reliable reports that exist.
With his sister as the visible absolute leader, Kim could possibly benefit greatly by staying invisible and holding all the powers of control from afar.
Let her be the gentler face of the 'new' North Korea, the tiny little girl who has to plead with the great powers who surround her to get what her people need so desperately.
It worked very well for Juan Perón in Argentina to allow his wife Evita to be the public face of his dictatorship. The Argentines loved Evita so much that even when she was dead, her memory allowed Perón to return to power after she was gone and continue his dictatorship until he died himself.
Evita made things very easy for her husband. He didn't have to bully his people around to get what he wanted from them.
That was in a fully developed western nation that has a tradition of democracy and an educated and sophisticated populace.
A nation like North Korea, un-educated, ignorant of the rest of the world, and accustomed to total dictatorship would be a push over for a similar arrangement.
