What makes you think the government will deny necessary care? My girlfriend is a veteran. She's never been denied at the local VA hospital. Her care is excellent. I'm curious as to where you guys dream this stuff up.
You're kidding right? You must not know that many vets then. Most I know don't even bother with the VA anymore.
Treatment can't be denied, but depending on the nature of the ER visit, payment certainly can be denied.
If you walk into the ER with a mosquito bite, you'll be handed the $500 bill.
That's funny ... when I had my third baby, I'm insured, so I was ok, but when I was checking in, a sign above the cube I was sitting in clearly stated they do not accept uninsured patients ... so it really depends on the hospital.

IIRC, public hospitals cannot deny treatment. Private hospitals can.
