My wife is very scared about pregnancy. She doesn't want the pain, doesn't, doesn't want to ruin the figure and she will not be able to quit smoking. She has proposed surrogacy. Do you think it is a good idea?
That might work if you have lots and lots of money to pay for it all.
Are you sure? We are talking about LOTS and LOTS of money---like famous movie star type or "old money" kind of money. Now, if your wife is a world famous supermodel, perhaps "not wanting to lose her figure" might be a legitimate reason, otherwise, "Nope".

Tell her to quit smoking even if she doesn’t have a child. I know plenty of moms who had two or three kids and they have great bodies.
Her body is gonna go through changes regardless. Eventually her assets may start pointing south a bit more. Even if she doesn’t have children.
It doesn't really matter - if she's convinced herself that she can't do what it takes then I doubt anyone else will be able to....but surrogates are expensive - do your research. And yes, look into nannies from Day 1 - she's not gonna like taking care of them either, I have a feeling.
A little fear is normal. The other things are weird. I would not have children with someone who will not even give up smoking for her own well-being.
