Where in SA can you go to apply for a LONG FORM birth certificate (which states the parents name on the certificate)

I found 2 links

but that $22 is for a SHORT term birth certificate and I need to apply for a long form birth certificate.

Do I need to go to the Vital and Statics records place and askt hem there??

The one I got for $24 at one of the city's service centers (Blanco and West) has the parents listed on it. I didn't know there was another type.

The short form, which you can get locally, has the parents names on it.

I think to get the long form you've got to order it from the vital statistics office in Austin, and check the box on the order form asking if the certificate will be used to obtain a passport.

SHort form indeed has both parents names on it, notes place, date, and time of birth and the state seal. It's valid for passports and anything else that requires a bc. I didn't even know a longer form existed in Texas. (I did have to submit other documents for my son's passport, but that's because he was born at home).

Oh yes there's a long form alright! That is required for a passport. When I needed a long form, I wrote on the application in all caps "LONG FORM NEEDED FOR PASSPORT" and then yellow-highlighted it, which gets their attention.

Weird. I've applied for passports for both my kids in the past 5 years, and no special "long" form or processing was required.The standard form you get from vital statistics for $22 is complete. Perhaps the standard is the "long" form of which you speak, but there's nothing special one need do to request it. Both were obtained from the vital statistics office downtown within 20 minutes.

You would be amazed at how fussy passport officers can be. The long and short birth certificates have the same information, just that the long one is a full page and the short form is half a page. Maybe they want to discourage fake birth certificates? I've been sent a short one only to have passport reject it. And no, Vital Stat is NOT going to refund your money.

A couple of things - 1) if you weren't born in Bexar then yes, it needs to go to Austin (or your county of birth) and 2) did it have the raised seal? not a copy?

We've had our own issues with the passport agency, but that was due to issues of fraud with midwives on the border 20+ years ago and them questioning any midwife-signed birth cert in Texas now (silly as the docs they accepted as proof of his citizenship were easily falsifiable and not relevant to birth in the US). But yeah, the form shown in the link of the OP, is the standard form, you take it to vital stats office downtown, Circle "passport," wait 20 minutes, pay your $ and go. I do believe my kids' birth certs are on regular 8x10 paper but I'd have to check. The key is the seal. And it has to have been printed within a certain time frame.
