This is clearly because they didn't include the entire St. Louis metro area and it's a conspiracy at the highest levels of the United States government.

It must be a conspiracy because apparently the city of St. Louis and the state of Missouri are now the same thing and nobody told me!

But that list ranks entire states, not just cities, so it's taking into account STL, KC, Springfield, Columbia, and all of rural Missouri. I would also assert that people in metropolitan areas have better access to higher quality mental health providers than people in rural areas, so if it weren't for the cities those numbers would probably be higher.

I'm from supposedly one of the most 'livable' countries in the world (Australia- currently ranked #2) and I was absolutely miserable there. I just don't buy into these ranking systems because it's all so subjective. What's livable vs miserable to person A may be the opposite to person B. I love St. Louis. I created a wonderful life here for myself once, and after a few years away, my husband and I are again creating a lovely, happy existence in the STL. The people who make up lists based on 'misery' or 'livability' look at numbers and percentages- not necessarily the human factors which come into play.
