I have been driven in Joberg in a rented car and there were no particular driving issues for my driving companion. There was only one evening where we got lost and it looked like the streets around us were unsafe but we had no problems just driving through the area.

I rented in Cape Town. No issues, besides learning how to drive on the other side of the road/car.

Which should take you no more than five minutes, even for a left-hand stick. (Clutch is still left-foot, in case you've always wondered.) The hardest part is learning to be an attentive driver again, paying attention the road and traffic and your car. Things you no longer do back home.
