It sounds more like a carb craving than a sweet tooth. I also "suffer" from this, you have to go cold turkey. Cut the carbs and the cravings disappear. Easier said than done. I did it though, I used the Atkins diet for a while and the cravings were gone, but once you finally break down and give in, watch out, it feels like you can never get your fill of carbs.

When you get a sweets craving just think about how much you hate being overweight. Go out and take a walk instead

I can't speak for you or anyone else except for myself;
what makes it or brakes it for me is to be strong when I'm in the food-store.
If I buy sugary stuff it's gonna come down my stomach pretty soon.

But if I'm I stay strong when I'm shopping for food, I won't have the option to eat it when there is none of it in my house.
Then I can swear over it etc, but if it's not there I physically can't eat it.

Have protein & fat instead. Make a Zone meal of 30/40/30 nutrition: 30% protein, 40% carb, 30% fat.

Go easy on yourself for the first week and don't try to diet at the same time that you cut out the sugar. Just eat normally and drink water, not soda, and no sweets or substitutes for that first week. And make sure you're not hungry when you go grocery shopping.

after some blood sugar fastng tests, turns out I am insulin resistant... (which is now being linked to PCOS)

being aware of why I was craving sugar is a big help.. it wasn't even a crave.. it was a feeling of "I need sugar"

I determined that my strongest sweets/carb cravings are around 3p - 4p each afternoon and after dinner each night.

It is helping me to have fruit on hand along with whole wheat bread (I usually toast it) or crackers during my "danger times." Also, I am moving towards a low-carb diet and for me, it works best if I leave my carb consumption until my last meal of the day.

If you're going low-carb, a couple of strawberries with a little cream makes a good dessert.

Protein will help a lot. Once you start to lose weight you won't want to eat sweets.
